Position Summary

The Diabetes Education Coordinator develops diabetes education programs that enhance quality of life for individuals with diabetes and their caregivers, including diabetes, exercise, and nutrition education. Clinical duties include consultations for inpatients and collaboration with team members to provide evidence-based care. The individual serving as the Coordinator demonstrates advanced knowledge of the lifelong process of managing a chronic disease and facilitating behavior change. The Coordinator works to maintain ADA (American Diabetes Association) recognition status and oversees the Center for Disease Control and Prevention’s Diabetes Prevention Program (DPP). The Coordinator serves as the chair of the Lakeland Regional Health Diabetes Advisory Board, develops and submits the annual plans for the outpatient Diabetes Self-Management Program and team member education as it relates to diabetes management.

  • Standard Work Duties: Diabetes Coordinator
  • The Diabetes Education Coordinator is responsible for developing a positive relationship with inpatients/outpatients and assisting the patient through the process of actively working towards optimal health and glycemic control through education.
  • In collaboration with nursing and medical staff, monitors the clinical care of patients and provides clinical support to improve patient care and outcomes, for both inpatient, outpatient, and across the continuum of care.
  • The Diabetes Coordinator conducts a thorough, individualized assessment of the person with or at risk of diabetes. The assessment process requires ongoing collection and interpretation of relevant data as well as the development of mutually acceptable goals.
  • The diabetes coordinator develops the DSME/T (diabetes self-management education/training) to attain the mutually defined goals to achieve desired outcomes. The plan integrates current diabetes care practices and established principles of teaching and learning. The plan is coordinated among the diabetes healthcare team members, the person with or at risk for diabetes, his or her family, significant others, relevant support systems and the referring provider.
  • The diabetes coordinator provides DSME/T according to the defined plan and desired goals and outcomes. Implementation may involve collaboration with other professional and community resources and services.
  • The Diabetes Coordinator evaluates and maintains an accurate record for each person with diabetes, aggregate outcome measures for the program and the quality of DSME/T.
  • The Diabetes Educator teaches use of blood glucose meter, insulin preparation, injection skills and techniques.
  • The Diabetes Coordinator works collaboratively with internal and external stakeholders to develop innovative projects and programs to educate community members about diabetes.
  • The diabetes coordinator develops and provides ongoing staff education including the direction and dissemination of diabetes information with the hospital wide Diabetes Liaison group.

Competencies & Skills

Essential:

  • Demonstrates familiarity with pathophysiology, epidemiology and clinical practice consistent with advanced practice nursing and diabetes education certification.
  • Fosters an inter-professional approach to safety, quality improvement, evidence-based practice, research, and translation of research into practice. Supports areas of responsibility as clinical expert resource for protocols, policies, and/or evidence-based order sets.
  • Provides diabetes support and care in a culturally-competent manner across the lifespan.
  • Applies current principles of teaching and learning and/or behavior change to facilitate self-management skills.
  • Contributes to the advancement of nursing by disseminating outcomes of research in best evidence-based practices. Pursues ongoing professional development.
  • Works with an interdisciplinary diabetes team to tailor interventions to individual self-management education including activity, blood glucose monitoring, carbohydrate counting, medication teaching, problem solving, survival skills, healthy coping, and prevention of acute and chronic complications.
  • Ability to assist patient and/or staff with use of insulin pump as needed during hospitalization.
  • Oversees program accreditation/recognition requirements for ADA Recognized Diabetes Program and CDC Diabetes Prevention Program.
  • Participates in professional organizations to address issues of concern to meet patients’ needs and to improve nursing practice and system effectiveness.
  • Integrates knowledge of governmental and regulatory opportunities/constraints to affect patient transitions across continuum of care.

  • Basic Cardiac Life Support

Other information:

Licenses Essential: Current license as Registered Nurse.

Certifications Essential: BLS

Certifications Preferred: Certification in Diabetes Education.

Experience Essential:

Diabetes Self-Management Education and Support ( DSMES) experience

Experience in developing curriculum, presentations and evaluation of programming.

Minimum five years of experience in diabetes education.

Academic or experiential preparation in areas of chronic disease care, patient education and/or program management.

Must be able to oversee the planning, implementation and evaluation of the diabetes programs.

Experience with facilitating behavior change.

Experience Preferred:

Life style coach through the CDC.

Experience diabetes education coordinator.
